    Requirements for Credit Card Payments.:
  • Certification Forms:Please have a copy of your certificate(s) and/or invoice ready to upload which is required for some of the payment request processes.
  • Resident Forms:Please have a copy of your invoice ready to upload when making payments.
    The Forms can be used for Mail-In Applications as well. See the details below:
  • Please complete the form and select Payment Type as "Mail-In Application"and then select "Submit" button.
  • Enclose a check or money order with the printed application (Payable to Commonwealth of PA).
  • Do not add documents, Please mail with your completed form and fee.
